Job summary

Qualifications

• Certified Nursing Assistant in good standing with the state.
• High school diploma or equivalent.
• Current CPR/BLS certification.
• At least one year of experience as a certified nursing assistant in long-term care.
• Ability to read, write, speak and understand English.
• Must be able to lift 35 pounds.

Responsibilities

• Provides supportive services to nurse(s) and other staff as needed and performs duties as assigned.
• Assists resident with activities of daily living per care plans and policies.
• Assists resident with lifting, turning, moving, positioning, and transporting.
• Coordinates dining room services at assigned mealtimes and documents meal intake.
• Delivers nutritional supplements and assists to ensure intake.
• Tracks resident weight and reports changes to nurse/supervisor.
• Monitors skin condition and reports issues to nurse/supervisor.
• Assists in toileting program activities and maintains flow sheets.
• Keeps team informed of factors interfering with tasks and resident condition.
• Reports changes in condition to nurse/supervisor.
