Job summary

Qualifications

Responsibilities

• Identify residents using wristband or photo card before treatments, meals, and tasks.
• Provide routine daily nursing care per care plan.
• Document entries clearly on flow sheets, charts, and notes.
• Report changes in resident condition to Nurse Supervisor/Charge Nurse.
• Maintain confidentiality of resident information and report incidents.
• Assist residents with daily living activities and mobility.
• Prepare residents for medical tests and appointments.
• Ensure clean and safe resident rooms and supplies.
• Participate in nursing shift reports and handoffs.
