Job summary

Illuminus is seeking a Certified Nursing Assistant at Lutheran Homes & Health Services, Fond du Lac, WI, to provide person-centered care under the supervision of a licensed nurse. This role supports residents with daily living activities and helps maintain dignity and comfort.

This position offers full-time, part-time, and PRN options with 8-hour shifts, weekend rotation, and a comprehensive benefits package including health plans, retirement, tuition reimbursement, and paid time off.

Qualifications

• At least 16 years of age.
• Be current on the Wisconsin Nurse Aide Registry.

Responsibilities

• Provides resident-centered personal care and mobility assistance according to care plans and scope of training while protecting resident rights and confidentiality.
• Accurately documents care in the medical record and CareTracker and reports resident needs to the team leader before the end of each shift.
• Follows safety, infection control, universal precautions, PPE, body mechanics, equipment-use, and organizational policies while maintaining a compassionate, professional approach.
• Maintains reliable attendance, supports staffing/on-call coverage, completes required training and education, and collaborates with all disciplines to uphold the organization’s mission, vision, and values.
