Qualifications

• Two requirements: At least 16 years of age.
• Be current on the Wisconsin Nurse Aide Registry.

Responsibilities

• Provides resident-centered personal care and mobility assistance according to care plans and scope of training.
• Accurately documents care in the medical record and CareTracker and reports needs to the team leader.
• Follows safety, infection control, universal precautions, PPE, body mechanics, equipment-use, and policies with compassion and professionalism.
• Maintains reliable attendance and supports staffing needs and training.
