Qualifications

• Certified Nursing Assistant in good standing with the state.

Responsibilities

• Provides supportive services to nursing staff and performs assigned duties.
• Assists residents with activities of daily living per care plans.
• Assists with transferring and moving residents safely.
• Coordinates dining room services and meal delivery.
• Delivers nutritional supplements and tracks intake.
• Records resident weights and reports changes.
• Monitors skin condition and reports issues.
• Assists with toileting program activities.
• Completes flow sheets and documents progress.
• Keeps supervisor informed of issues affecting care.
• Reports changes in resident condition to nurse.
• Other duties as assigned.
