• Performs initial and ongoing data collection on patients/residents under the direction of the RN/LPN.
• Implements plan of care developed by the RN based on patient/resident needs.
• Provides direct patient care and reports condition to RN or LPN.
• Ensures resident safety and initiates corrective action as needed.
• Documents results of patient/resident care per policies.
• Other duties as assigned.
