Ashton Creek Health and Rehabilitation in Fort Wayne, IN is hiring CNAs for full-time and part-time roles on the 2nd shift with a $4 shift differential. You will assist residents with daily care, support feeding and mobility, and maintain patient rooms while ensuring safety and HIPAA compliance.

Qualifications

• Must have ISDH CNA certification in good standing.
• High school diploma or GED required.
• Basic computer skills (Excel, Word) required.
• Excellent interpersonal and communication skills.

Responsibilities

• Assist residents with bathing, toileting, grooming, and dressing.
• Feed residents, provide bedside water, and transfer to dining areas per care plans.
• Respond promptly to call lights and resident requests.
• Maintain resident rooms and observe changes in appetite or condition.
• Keep patient information confidential and document vitals and care provided.
• Ensure safety, infection control, and a clean work environment.
• Follow facility policies and HIPAA compliance.
