Job summary

Xenia Health and Rehab in Xenia, OH is seeking a Certified Nursing Assistant to provide direct resident care under licensed staff supervision. Responsibilities include ADL assistance, safety, and comfort, contributing to quality of life for residents.

The CNA works with the nursing team to follow care plans, document progress, monitor weight and skin condition, and assist with meals and daily activities as needed.

Qualifications

• Certified Nursing Assistant in good standing with the state.
• High school diploma or equivalent.
• Current CPR/BLS certification.

Responsibilities

• Assist residents with activities of daily living per care plans and policies.
• Coordinate dining room services: setup, delivery, feeding and documentation of intake.
• Weigh residents and report weight changes to nurse/supervisor.
• Monitor skin condition and report any issues to nurse.
• Document progress and communicate changes to supervisor as directed.
