Job summary

Polaris Rehabilitation & Care Center is seeking a compassionate CNA to join our nursing team. You will provide hands-on care, assist with daily living activities, and work closely with nurses to ensure residents receive safe, individualized care.

Qualifications

• Current, active Wyoming CNA certification in good standing.
• Nurse Aide Registry clearance without disqualifying findings.
• Experience in skilled nursing or related healthcare settings preferred but not required.
• Strong communication and interpersonal skills.
• Ability to follow individualized resident care plans and nursing direction.
• Dependable team member who can prioritize tasks in a fast-paced setting.
• Commitment to compassionate, resident-centered care.

Responsibilities

• Assist residents with activities of daily living (ADLs) such as bathing, dressing, grooming, toileting, feeding, and personal hygiene.
• Record vital signs accurately (temperature, pulse, respirations, blood pressure).
• Monitor residents for changes in condition and report to supervising nurse.
• Assist with transfers, ambulation, and mobility according to care plans and safety procedures.
• Respond promptly to call lights and resident requests.
• Provide meal assistance and feeding support as needed.
• Deliver routine skin, incontinence, oral, and personal care within CNA scope.
• Document care and services per facility procedures.
