Job summary

Swltc in Hobart, OK is seeking compassionate CNAs to provide direct patient care in a skilled nursing facility. You will assist residents with ADLs, monitor vital signs, support mobility and transfer needs, and document care while upholding dignity and safety.

The ideal candidate holds an active CNA certification in Oklahoma, has strong communication and teamwork skills, and demonstrates a patient-centered approach to care for elderly and medically complex residents.

Qualifications

• Active Certified Nurse Aide (CNA) certification in the state.
• Experience in skilled nursing facility, long-term care, or rehabilitation preferred.
• Ability to lift, transfer, and assist residents as needed.
• Strong communication and teamwork skills.
• Compassionate, patient-centered approach to care.

Responsibilities

• Assist residents with activities of daily living (ADLs) including bathing, dressing, grooming, toileting, and feeding.
• Take and record vital signs such as blood pressure, temperature, pulse, and respiration.
• Assist residents with mobility, transfers, and repositioning to prevent pressure injuries.
• Observe and report changes in resident condition to the licensed nurse.
• Maintain accurate documentation of care provided.
• Support residents with meals, hydration, and nutritional needs.
• Provide emotional support and maintain a respectful and compassionate environment.
• Ensure residents’ rooms and common areas remain clean, safe, and organized.
• Follow infection control policies and safety procedures.
• Assist nursing staff with other duties as assigned.
