Job summary

Workstream is seeking a field-based caregiver to travel to clients’ homes and provide supportive personal and household services under a written plan of care.

Must obtain CNA or CHHA certification and hold a valid driver’s license; willing to travel 100% and work on the road and in members’ homes. Experience as PCA/CNA/CHHA is preferred but not required.

Responsibilities

• Traveling to and from clients’ homes.
• Providing supportive household services that may include light housekeeping, laundry, grocery shopping, preparing meals (sometimes for medically prescribed diets etc. that are outlined in the member’s written plan of care).
• Documenting observations and activities.
• Performing care and services with minimal or no client/family complaints.
• Assisting clients with personal care as directed by their written plan of care (bathing, dressing, grooming).
• Taking oral and written instructions.

Duties and Responsibilities:

1. Traveling to and from clients’ homes.
2. Providing supportive household services that may include light housekeeping, laundry, grocery shopping, preparing meals (sometimes for medically prescribed diets etc. that are outlined in the member’s written plan of care.
3. Documenting observations and activities.
4. Performing care and services with minimal or no client/family complaints.
5. Assisting clients with personal care as directed by their written plan of care. These tasks may include bathing, dressing, or grooming.
6. Taking oral and written instructions.
7. Must have telephone and dependable transportation.

The applicant must be able to communicate

effectively (written and verbal) with:

Scheduling coordinators regarding dates and times of shifts as well as the health of members while maintaining HIPAA regulations for confidentiality.

Members that are hard of hearing, linguistically impaired, and/or emotionally distraught. This may require the use of assistive devices.
