Embrace Hospice seeks a CNA to provide compassionate, patient-centered care in San Antonio, TX. The role emphasizes 1:1 interaction, dignity, and support for patients and families.

The CNA will assist with daily activities, monitor patient conditions, document care, and coordinate with the interdisciplinary team to ensure high-quality end-of-life care. CPR certification is required and guidelines vary by state.

Qualifications

• Must complete 75 hours of classroom and supervised training.
• CPR certification current.
• 18+ years of age.
• Able to read and follow written instructions.
• Comfortable providing care to terminally ill.

Responsibilities

• Provide 1:1 patient care and personal care.
• Monitor changes in patient condition and report.
• Assist with mobility, transfers, ROM, and daily living tasks.
• Maintain accurate, timely documentation.
• Collaborate with hospice interdisciplinary team.

Responsibilities

As a Hospice CNA You will:Personal Care : Help with bathing, dressing, toileting, mobility, and repositioningMonitor & Report:Observe and report changes in the patient's conditionMobility:Assist with ambulation, transfers, ROM exercises and safe equipment useMedication Assistance : Help with self-administered medications per state regulationsEmotional Support:Offer companion​ship and comfort to patients and families.Light Housekeeping:Change linens, assist with meals and maintain a clean space.Documentation:Maintain accurate and timely documentationTeam Participation:Collaborate with hospice interdisciplinary team

Qualifications

Meet the training requirements of the state of practiceMinimum of six months experience including providing personal careMust be 18+ years of ageComplete a minimum of 75 hours of classroom and supervised practical training, 16 of the 75 hours of classroom must be complete prior to a mandatory 16 hours of practical trainingPossess and maintain current CPR certificationAbility to read and follow written instructions and document care givenUnderstands hospice philosophy, comfortable providing specialized care to the terminally illAbility to work with little direct supervisionAbility to handle death/dyingStrong oral and written communication skillsGood Organizational Skills
