Responsibilities
  • Performs personal care needs as assigned, attending to patient requests promptly. Reports to the Case Manager if unable to perform a task.
  • Provides positive communication with patients and families, maintaining confidentiality and ethical standards.
  • Ensures patient safety, using equipment properly (foot stools, side rails, O2).
  • Provides personal care including baths, oral hygiene, shampoos, and linen changes.
  • Assists with dressing, feeding, and mobility as instructed.
  • Records vital signs and observes skin condition to prevent breakdown.
  • Reports patient condition changes to the Case Manager.
  • Supports patients with medication reminders, light housekeeping, and other care activities.
  • Maintains a clean and orderly patient environment, adhering to care standards.
  • Participates in quality improvement activities and respects patient and family needs.
Qualifications
  • Meets state training requirements, including 75 hours of classroom and practical training.
  • Minimum six months experience providing personal care.
  • At least 18 years old, with the ability to read, follow instructions, and document care.
  • Understanding of hospice philosophy and comfort with end-of-life care.
  • Current CPR certification, with good personal hygiene and tactful communication skills.
  • Successfully completes probationary period demonstrating competence.
