Location: Fond Du Lac Area

Schedule: Mon-Fri, no weekends or holidays, no on-call, day shift hours

How You Will Contribute:

1. Coordinate with the assigned nurse to provide safe patient care and report on the patient's condition.
2. Perform personal care, take vitals, communicate with families, and complete other duties as assigned.
3. Travel between patients' homes and work as part of an interdisciplinary team supporting patients and families during end-of-life care.

Qualifications:

• Meet state training requirements
• Minimum six months experience including personal care
• At least 18 years old
• Complete 75 hours of classroom and supervised practical training (16 hours of classroom prior to 16 hours of practical training)
• Current CPR certification
• Ability to read, follow instructions, and document care
• Understanding of hospice philosophy and comfort with caring for terminally ill patients
• Ability to work independently
• Ability to handle death/dying
• Strong oral and written communication skills
• Good organizational skills
