Location: Fond Du Lac Area

Schedule: Mon-Fri, no weekends or holidays, no on-call, day shift hours

How You Will Contribute

• Coordinate with the assigned nurse to provide safe patient care and report on the patient’s condition.
• Perform personal care, take vitals, communicate with families, and other duties as assigned.
• Travel between patients and work as part of an interdisciplinary team supporting families and patients during end-of-life care.

Qualifications

• Meet state training requirements
• Minimum six months experience providing personal care
• At least 18 years old
• Complete 75 hours of classroom and supervised practical training, with 16 hours of classroom prior to 16 hours of practical training
• Current CPR certification
• Ability to read and follow written instructions and document care
• Understanding of hospice philosophy and comfort providing specialized care to the terminally ill
• Ability to work with little supervision
• Ability to handle death/dying
• Strong oral and written communication skills
• Good organizational skills
