Coverage area: Can be asked to travel throughout the territory

Schedule: PRN

How you will contribute (not limited to)

You will make a difference by coordinating with the assigned nurse to provide safe patient care, report on the patient’s condition, perform personal care, take vitals, communicate with families, and other duties as assigned. You will travel between patients and work as part of an interdisciplinary team supporting families and patients during their end-of-life journey. Qualifications

• Meet the training requirements of the state of practice
• Minimum of six months experience including providing personal care
• At least 18 years of age
• Complete a minimum of 75 hours of classroom and supervised practical training, with 16 hours of classroom prior to 16 hours of practical training
• Possess and maintain current CPR certification
• Ability to read and follow written instructions and document care given
• Understand hospice philosophy and be comfortable providing specialized care to the terminally ill
• Ability to work with little supervision
• Ability to handle death/dying
• Strong oral and written communication skills
• Good organizational skills
