Job summary

Adoration Health in Dublin, GA is seeking a Home Health CNA to provide patient-centered, 1:1 care under RN supervision. You will document care using HomeCare HomeBase EMR and assist with personal care and home management tasks in a supportive team environment.

The role offers Monday–Friday, 8am–5pm coverage in Laurens, with opportunities for advancement and a range of health benefits including medical, dental, vision, and a 401(k) with company matching.

Qualifications

• CNA or Home Health Aide certification as required by state licensure.
• Must be at least 18 years old and have a valid driver's license.
• Ability to lift up to 75 lbs and perform basic personal care tasks.

Responsibilities

• Provide patient-centered care under the supervision of a Registered Nurse.
• Complete proper documentation using HomeCare HomeBase EMR.
• Assist clients with personal care or home management tasks.
• Assist with basic hygiene, feeding, ambulation, and medical monitoring.
• Home management includes basic housekeeping, shopping and related tasks.

Responsibilities

• This would include assistance with basic personal hygiene and grooming, feeding and ambulation, medical monitoring, and other basic health care related tasks
• Home M anagement includes tasks that include basic housekeeping, shopping and other related housekeeping tasks

Qualifications

• Minimum age requirements are 18 years
• Valid driver's license, with satisfactory driving record, as defined by agency policy
• Certification, if required by state licensure as a CNA or Home Health Care Aide in the state in which the aide is to provide care
• Must be able to utilize proper body mechanics while lifting and may be expected to lift up 75 lbs.; must be able to sit, stand, and walk for periods of time and bend, stoop, push, pull, and reach
