Job summary

Adoration Health in Auburn, AL seeks a passionate Home Health CNA to deliver 1:1 patient-centered care in the home. This PRN role offers flexible scheduling and the opportunity to make a meaningful impact on patients and families.

Responsibilities include providing care under RN supervision, accurate documentation in HomeCare HomeBase, and assisting with personal care and basic home management tasks. A valid CNA license and driver’s license are required.

Qualifications

• Must be a CNA with valid licensure in the state.
• Ability to provide patient-centered care.
• Excellent communication with patients and families.
• Must be able to lift up to 75 lbs. and perform daily tasks.

Responsibilities

• Provide patient-centered care under the supervision of a Registered Nurse.
• Complete proper documentation using HomeCare HomeBase EMR.
• Assist clients with personal care or home management tasks.
• Assist with basic hygiene, grooming, feeding, ambulation, and health monitoring.
• Home management tasks include basic housekeeping and shopping.

Responsibilities

• Provide patient-centered care under the supervision of a Registered Nurse

• This would include assistance with basic personal hygiene and grooming, feeding and ambulation, medical monitoring, and other basic health care related tasks
• Home M anagement includes tasks that include basic housekeeping, shopping and other related housekeeping tasks

Qualifications

• Minimum age requirements are 18 years
• Valid driver's license, with satisfactory driving record, as defined by agency policy
• Certification, if required by state licensure as a CNA or Home Health Care Aide in the state in which the aide is to provide care
• Must be able to utilize proper body mechanics while lifting and may be expected to lift up 75 lbs.; must be able to sit, stand, and walk for periods of time and bend, stoop, push, pull, and reach
