Job summary

Apple Rehab Farmington Valley in Plainville, CT is seeking a compassionate CNA to join our family‑owned team. You will assist residents with bathing, grooming, mobility, and daily living activities, following facility policies and care plans.

We offer shift differentials for 2nd/3rd shifts and weekends, multiple full/part-time options, per diem, and a comprehensive benefits program including health, dental, vision, 401K, and career development.

Qualifications

• Must hold current Certified Nursing Assistant (CNA) certification in good standing.
• No history of patient/resident abuse as per State Registry.
• Proficient in reading, writing and following oral and written instructions.
• Strong attention to detail, excellent follow-through, and ability to prioritize multiple tasks.
• Team player with good cooperation and interpersonal skills.
• Demonstrates patience, tact, enthusiasm, and a positive attitude towards residents and their support networks.

Responsibilities

• Assists residents in daily bathing, grooming, dental care, bowel and bladder functions, dressing, walking and wheelchair use.
• C.N.A. documents personal care given according to facility policies.
• Provides nursing functions as directed including perineal care, making beds, turning residents, measurements and charting.
• Observes and reports changes in residents’ condition to supervisor.
• Records all necessary charting entries and responds to call lights and emergencies.

Job Opportunity:

Differentials Offered:

• Shift Differentials for 2nd and 3rd shifts and weekends
• Additional Shifts Differential: Extra $5.00/hour for any shifts worked outside scheduled hours

Available Shifts:

• Full-Time 7-3
• Part-Time 7-3 and 11-7
• Per Diem

Responsibilities:

Assists residents in daily bathing, grooming, dental care, bowel and bladder functions, dressing, walking and wheelchair use, positioning in and out of bed. C.N.A. Certified Nurse’s Assistant documents personal care given according to facility policies. Provides nursing functions as directed including perineal care, making beds, turning residents, measures and records temperature, weight, pulse and respiration. C.N.A. Certified Nurse’s Assistant observes and reports to supervisor changes in residents’ condition, including any skin breakdowns. Reports immediately all accidents or injuries. Records all necessary charting entries. Answers all call lights regardless of assignment. Responds to emergency situations calmly and efficiently.

Qualifications:

• Must hold current Certified Nursing Assistant (CNA) certification in good standing.
• No history of patient/resident abuse as per State Registry.
• Proficient in reading, writing, and following oral and written instructions.
• Strong attention to detail, excellent follow-through, and ability to prioritize multiple tasks.

Additional Expectations:

Per diem staff are expected to fulfill (2) weekend shifts monthly to meet requirements.
