Southern HealthCare Management is seeking an individual to assist the Activity Manager in delivering a program of activities aimed at supporting the physical, mental, and psychosocial well-being of patients. The role involves coordinating volunteers, helping implement activity plans, documenting attendance, and supporting overall patient engagement within the facility.

Qualifications

• High School Diploma or equivalent; 1–3 months related experience and/or training.
• Active Certified Nursing Assistant (CNA) certification.
• 2 years of experience in a social or recreational program within the last 5 years, one of which was full-time in a therapeutic activities program.
• Proficient in the use of a personal computer.

Responsabilités

• Assist the Activity Manager in providing activities for patients.
• Assist with individual volunteers and volunteer programs as needed.
• Assist with planned activity programs to meet patients' needs and interests.
• Complete attendance/participation records for individuals and groups.
• Assist in assessments, care plans, and information gathering for activities.
• Escort patients to and from activities.
• Develop the Activity calendars.
• Provide personal care when needed and support daily living activities.
• Record vital signs and weight as assigned.
• Document care per policy and regulations.
• Maintain patient confidentiality and protect the facility.
