• Monitor residents and report changes to Charge Nurse or Director of Nursing.
• Assist with mobility, transfers, and ambulation to support physical well-being.
• Maintain communication with residents, families, and team members.
• Document care and condition changes in line with policies.
• Ensure a clean, safe environment following infection control protocols.
• Provide companionship and engage residents in activities to support mental well-being.
• Support residents' spiritual offerings when chosen.
