Job summary

The Care Team Hospice is seeking a compassionate Certified Nursing Assistant (CNA) for the 2nd shift. Start at 12-2p, finish around 8-10p; no nights, no weekends, and no on-call. Serve patients within about an hour of the branch and participate in comprehensive hospice care.

Qualifications

• Active CNA certification required.
• Minimum CNA experience; hospice/home health preferred.
• Ability to perform personal care and maintain documentation.

Responsibilities

• Provide personal care, bathing, grooming, and comfort measures as per care plan.
• Observe and report changes in patient condition to supervising nurse.
• Support patients and families with dignity and respect.
• Assist with mobility and activities of daily living safely.
• Document all care in the electronic medical record.
