Job summary

The Oaks Healthcare Center in Poteau, OK is seeking compassionate care staff for the Evening shift in hospice services. You will follow RN-directed care plans, assist with personal care and mobility, and help maintain a safe, comforting home environment for patients.

Qualifications include a high school diploma or GED and current CNA/CHHA/LTC licenses with a valid driver’s license and auto liability insurance. Benefits include BCBS, dental/vision, PTO, and mileage reimbursement.

Responsibilities

• Follows written instructions for patient care as prepared by an RN or Clinical Director in the hospice plan of care.
• Assists patients with personal care and transfers to preserve hygiene and safety.
• Performs simple procedures as an extension of therapy services (ambulation, ROM, exercises).
• Reports changes in patient condition and needs to supervisor or RN.
• Maintains records of services performed and patient condition.
• Establishes and maintains a good working environment with patient/family and hospice staff.
• Participates in patient care conferences to provide input for continuity of care.
• Performs household services essential to home health care.

• Performs simple procedures as an extension of therapy services, such as ambulation, ROM and exercise.
• Reports changes in patient’s condition and needs to supervisor or RN.
• Maintains record of services performed and of apparent condition of patient.
• Establishes and maintains a good working environment with the patient/family and co-workers of the Hospice.
• Participates in patient care conferences to provide input requiring continuum of patient care.
• Perform household services essential to health care at home.
