Job summary

West River Care in Massachusetts is seeking compassionate Home Health Aides to provide hospice services in the Acton/Concord area and Newton area. Starting at $25/hour with guaranteed at least 24 hours per week.

You will assist patients with personal care, maintain safety, support mobility and nutrition, provide companionship, and document care while collaborating with the interdisciplinary team to meet emotional needs of families.

Qualifications

  • High School diploma or GED required
  • Proof of Massachusetts certification
  • Minimum of one year of direct care experience in home care
  • Hospice experience is highly desirable
  • Ability to communicate professionally with coworkers, patients and families

Responsibilities

  • Ensure quality and safe delivery of hospice services to patients, families, and caregivers
  • Assist patients with personal care activities as described in the plan of care
  • Maintain a safe environment for patients and use equipment safely
  • Provide companionship and respite for families/caregivers
  • Document care provided in a timely manner per agency policy
  • Collaborate with the IDG to meet emotional needs of families and ensure continuity of care
  • Support independence and assist with light housekeeping and meal preparation
  • Complete required in-service training including dementia training
  • Participate in team meetings and quality improvement projects

2 Positions Available -- (1) Acton/Concord & Surrounding Area (1) Newton & Surrounding Area

Starting at $25/hour, guaranteed at least 24 hours per week!

Essential Duties and Responsibilities:

  • Ensure quality and safe delivery of hospice services to patients, families, and caregivers
  • Perform or assist patients with personal care activities described in the plan of care, including but not limited to hygiene, mobility and elimination. If not able to perform a task, report to the primary RN case manager immediately
  • Perform and maintain a safe environment for patients
  • Use equipment safely and properly
  • Provide universal for conscience and all contacts with patients
  • Perform or assist patient/family with home maintenance activities as described in the care plan, including but not limited to light housekeeping and meal preparation
  • Perform support activities as prescribed in the care plan including but not limited to supporting independence, providing companionship and providing respite for the family/caregivers
  • Respect the autonomy and right to confidentiality of all patients and families/caregivers
  • Monitor adequate fluid and nutrition intake, temperature, pulse, respirations, skin and basic elements of body functions and record/report appropriately
  • Practice safe transfers, ambulation and normal range of motion and position techniques
  • Identify the need for intervention by other members of the interdisciplinary team on an ongoing basis and report to primary RN case manager
  • Report on the effectiveness of the care plan
  • Document care provided in response with patient and family/caregiver submit documentation in a timely manner per agency policy
  • Assist patient, family/caregiver and other team members and providing continuity of care
  • Work in cooperation with IDG to meet the emotional needs of the patient's family/caregiver
  • Manage a caseload and deliver care as prescribed in the care plan use supplies and equipment effectively and efficiently
  • Complete a minimum of 12 hours of in-service per year to include dementia training
  • Actively participate in team meetings and contribute positively to the patient's care planning
  • Participate in QAPI, special projects and perform other duties as assigned

Desired Qualifications:

  • High School graduate or GED required
  • Proof of certification by state of Massachusetts
  • A minimum of one year of direct care experience in an institute of home care setting
  • Experience in hospice is highly desirable
  • Ability to effectively communicate in a professional and composed manner with coworkers, patients and families

Job Performance Criteria:

  • Sit and stand during most of the workday
  • Should be able to stand and slash walk for at least half an hour at a time
  • Communicate verbally and in writing with the interdisciplinary team patients, families and referral sources
  • Must demonstrate skills and written slash pro competency prior to working independently with the patient
  • Sufficient documentation for having passed a skill test
