Job summary

Northeast Michigan Community Mental Health Authority in Alpena, MI seeks a Community Living Support (CLS) Worker to assist individuals served in achieving IPOS goals and community participation. You will provide support, monitor health and safety, and coordinate with teams, families, and providers.

This role requires a valid driver's license, ability to lift up to 50 pounds, and availability to work evenings, weekends, and holidays. HS diploma or GED preferred.

Qualifications

• High School diploma or GED preferred.
• Knowledge of intellectual/developmental disabilities (I/DD) and mental health issues desirable.
• Ability to communicate effectively with individuals, families, natural supports, and providers.
• Lived experience with mental illness, intellectual/ developmental disabilities, and/or substance use disorders valued.

Responsibilities

• Provide support and services to individuals served as identified in the IPOS.
• Adhere to agency policy and procedures for assisting individuals with medication setup and/or delivery.
• Assist in finding and maintaining volunteer opportunities if desired.
• Work effectively with providers, families, natural supports, and case managers concerning assessments, treatment planning, and service implementation.
• Will assist with preserving the health and safety of individuals served to support the most integrated and independent setting.
• Ability to prepare progress notes, reports, and required documentation according to agency clinical documentation standards.
• Demonstrates knowledge of universal precautions, standards of practice, and emergency protocol.
