You will design and implement individualized behavioral health plans, providing direct care, counseling, and collaboration with a broad care team.

The role emphasizes trauma-informed care, crisis support, and recovery-focused outcomes, with a $2,000 sign-on bonus and DailyPay available.

Qualifications

• Masters degree plus applicable Louisiana clinical credential (PLPC/LPC/LMSW/LCSW/LISW).
• Minimum two years of experience in behavioral health settings.
• Valid drivers license and access to a personal vehicle.
• Ability and willingness to travel for community-based services.
• Strong collaboration within a multidisciplinary ACT team.

Responsibilities

• Clinical assessment and direct care using validated tools.
• Develop measurable treatment goals and updated individualized plans.
• Coordinate with therapists, case managers, physicians and other providers.
• Provide crisis intervention and trauma-informed care when needed.
• Maintain thorough clinical documentation and ensure confidentiality.
• Support participants toward independence and recovery goals.
