Job summary

Horizon Health and Wellness, Inc. is seeking a licensed clinician to provide therapy to children, families, and adults in a central Arizona setting. You will perform assessments, diagnosis, treatment planning, and case management, while maintaining accurate clinical records.

A collaborative, multi-disciplinary team approach is essential. Requirements include a Master’s degree in counseling or related field, AZBBHE licensure eligibility, and 21+ years of age.

Qualifications

• Must meet eligibility for AZBBHE licensure.
• Master's degree in Counseling, Marriage & Family, Psychology, Social Work, or related field.
• Experience with best-practice therapeutic approaches.
• Must be 21 years or older.
• Valid driver license with liability coverage if driving on agency time.
• CPR/First Aid certification or training provided if needed.
• Fingerprint clearance, pre-employment drug screen, and background check.
• TB screening with proof of being free of infectious TB.

The therapy shall include assessments, evaluation, diagnosis, treatment and case management.
