Job summary

Elara Caring is seeking a Clinical Team Manager (RN or PT) to lead our home health team in Indiana. You will oversee patient care services, staff assignments, care coordination, and ensure timely start of care while maintaining regulatory compliance.

You'll review plans of care, mentor clinicians, manage OASIS reviews, and partner with the Branch Director to monitor performance metrics. A valid RN or PT license and 2 years in home health are required.

Qualifications

• Associate degree in Nursing or graduation from an APTA-approved PT program.
• Current, unrestricted RN or PT license for Indiana.
• 2 years home health experience as a registered nurse in Home Health or Hospice; supervisory experience preferred.

Responsibilities

• Oversee patient care services, staff assignments, referrals, and care coordination.
• Plan and coordinate Home Health services with patients, families, physicians, providers, vendors, and facilities.
• Ensure timely start of care and compliance with regulatory requirements; review Plans of Care and physician orders.
