Elara Caring is seeking a Clinical Team Manager to oversee patient care services, staff scheduling, and care coordination across home health settings. The role emphasizes regulatory compliance, OASIS reviews, and hands-on leadership to ensure high-quality patient outcomes.

Ideal candidates hold an RN license, have at least 2 years of home health or hospice experience, and at least 1 year of supervisory leadership. Proficiency with Homecare Homebase, Workday, and Microsoft Office is required.

Competenze

• Associate degree in a Nursing related field; Bachelor preferred.
• 2 years of home health or hospice experience.
• 1 year of supervisory experience.
• Current, unrestricted RN license for the state of work.
• Proficiency in Homecare Homebase, Workday, and Microsoft Office.
• Ability to lift, push, pull, and move up to 50 pounds.
• Valid driver's license or reliable transportation for travel between patient homes.

Mansioni

• Oversee patient care services, staff assignments, referrals, and care coordination.
• Plan and coordinate Home Health services with patients, families, physicians, providers, vendors, and facilities.
• Ensure timely start of care and regulatory compliance.
• Review Plans of Care and physician orders for clinical appropriateness.
• Conduct regular OASIS reviews for accurate documentation.
• Provide coaching and support to direct care staff and assist with interviewing.
• Collaborate with Branch Director to monitor KPIs and support operations.
