Job summary

Elara Caring is seeking a Clinical Team Manager in Indiana to oversee patient care services, staff assignments, and care coordination across Home Health operations. The role requires supervising clinical staff, ensuring timely starts of care, and maintaining compliance with regulatory standards.

The successful candidate will lead care teams, review Plans of Care, and coordinate with physicians, vendors, and facilities while driving quality and performance metrics.

Qualifications

• Associate degree in a Nursing related field or Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
• Current, unrestricted RN or PT license valid for the state of work.
• 2 years home care experience as a registered nurse in a Home Health or Hospice environment.
• 1 year of supervisory experience preferred.
• Ability to quickly become proficient in enterprise applications such as Homecare Homebase, Oasis, Workday, and have knowledge in Microsoft Office.

Responsibilities

• Oversee patient care services, staff assignments, referrals, and care coordination to ensure patient needs are consistently met.
• Plan and coordinate Home Health services with patients, families, physicians, providers, vendors, and facilities.
• Ensure timely start of care and ongoing compliance with applicable regulatory requirements.
• Review and approve individualized Plans of Care, physician orders, and proposed changes for clinical appropriateness.
• Conduct regular OASIS reviews to support accurate documentation, compliance, and quality patient care.
• Provide guidance and coaching to direct care team members and assist with interviewing and hiring clinical staff.
• Partner with the Branch Director or Administrator to monitor key performance metrics and support clinical operations, including field work as needed.

Education

Associate degree in Nursing related field or Graduate of APTA approved PT program

2 years home health RN experience

1 year supervisory experience

Proficiency in Homecare Homebase, Oasis, Workday, MS Office
