Job summary

Elara Caring seeks a Clinical Team Manager in Illinois to oversee patient care services, staff assignments, referrals, and care coordination for Home Health and Hospice settings.

You will ensure timely start of care, review plans of care and physician orders, and conduct OASIS reviews to maintain high-quality patient care. Leadership and coaching are expected to drive clinical excellence.

Qualifications

• Associate degree in a Nursing related field required.
• 2 years home care experience as an RN in a Home Health or Hospice environment.
• 1 year of supervisory experience preferred.
• Current, unrestricted RN license valid for the state of work.
• Ability to quickly become proficient in enterprise applications such as Homecare Homebase, Workday, and have knowledge in Microsoft Office.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.

Responsibilities

• Oversee patient care services, staff assignments, referrals, and care coordination.
• Plan and coordinate Home Health services with patients, families, physicians, providers, vendors, and facilities.
• Ensure timely start of care and ongoing compliance with applicable regulatory requirements.
• Review and approve individualized Plans of Care, physician orders, and proposed changes for clinical appropriateness.
• Conduct regular OASIS reviews to support accurate documentation, compliance, and quality patient care.
• Provide guidance and coaching to direct care team members and assist with interviewing and hiring clinical staff.
• Partner with the Branch Director or Administrator to monitor key performance metrics and support clinical operations, including field work as needed.
