Job summary

Elara Caring is seeking a Clinical Team Manager in Akron, Ohio to guide clinicians, coordinate patient care, and support branch leadership. You will oversee care coordination, reviews of plans of care, and ensure compliance with regulatory requirements.

The role combines hands-on clinical knowledge with team coaching and care coordination, including interviewing and hiring responsibilities and field work as needed.

Qualifications

• Associate degree in nursing-related field required.
• At least 2 years of home care experience as an RN in home health or hospice.
• Current, unrestricted RN license valid in Ohio.
• 1 year of supervisory experience preferred.
• Ability to quickly become proficient in Homecare Homebase and Workday, with working knowledge of Microsoft Office.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• A valid driver’s license or reliable transportation to travel between patient homes and assigned work locations.

Responsibilities

• Lead patient care coordination, overseeing staff assignments, referrals and services.
• Keep care moving by coordinating with patients, families, physicians, providers, vendors and facilities for timely starts of care.
• Guide clinical decisions by reviewing and approving Plans of Care, physician orders, and proposed changes for clinical appropriateness.
• Support quality and compliance by conducting OASIS reviews and ensuring documentation meets regulatory requirements.
• Coach your team by providing clinical guidance and staff development, and participating in interviewing and hiring.
• Partner with branch leadership to monitor performance metrics and support daily operations, including field work as needed.
