Job summary

Elara Caring is seeking a Clinical Team Manager in Illinois to lead home health services, coordinate patient care, and ensure regulatory compliance. This role focuses on staff supervision, plan-of-care review, and field support to maintain high-quality patient outcomes.

The position requires RN licensure, 2 years in home care, and at least 1 year in supervision, with proficiency in Homecare Homebase and Workday. A Bachelor’s degree is preferred, and a driving requirement may apply.

Qualifications

• 2 years of home care experience in Home Health or Hospice environment required.
• 1 year of supervisory experience required.
• Current, unrestricted RN license valid for the state of work required.
• Associate degree in a Nursing related field, Bachelor preferred.

Responsibilities

• Oversee patient care services, staff assignments, referrals, and care coordination to ensure patient needs are met.
• Plan and coordinate Home Health services with patients, families, physicians, providers, vendors, and facilities.
• Ensure timely start of care and ongoing compliance with applicable regulatory guidelines.
• Review and approve individualized Plans of Care, physician orders, and proposed changes for clinical appropriateness.
• Conduct regular OASIS reviews to support accurate documentation and quality patient care.
• Provide ongoing guidance, coaching, and support to direct care team members and assist with interviewing and hiring clinical staff.
• Partner with the Branch Director or Administrator to monitor key performance metrics and support clinical operations, including field work as needed.

What is Required?

• 2 years of home care experience in Home Health or Hospice environment required
• 1 year of supervisory experience required
• Current, unrestricted RN license valid for the state of work required
• Associate degree in a Nursing related field, Bachelor preferred
• Ability to quickly become proficient in enterprise applications such as Homecare Homebase, Workday, and have knowledge in Microsoft Office
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.

• The base salary for this position is $82,000 to $103,000, based on the company’s good faith estimate at the time of posting.
• Actual pay will be determined based on factors such as education, experience, skills, and internal equity.
