Job summary

Elara Caring is seeking a Clinical Team Manager to oversee patient care services, staff scheduling, and care coordination across home health settings. The role emphasizes regulatory compliance, OASIS reviews, and hands-on leadership to ensure high-quality patient outcomes.

Ideal candidates hold an RN license, have at least 2 years of home health or hospice experience, and at least 1 year of supervisory leadership. Proficiency with Homecare Homebase, Workday, and Microsoft Office is required.

Qualifications

• 2 years of home health or hospice experience.
• 1 year of supervisory experience.
• Current, unrestricted RN license for the state of work.
• Proficiency in Homecare Homebase, Workday, and Microsoft Office.
• Ability to lift, push, pull, and move up to 50 pounds.
• Valid driver's license or reliable transportation for travel between patient homes.

Responsibilities

• Oversee patient care services, staff assignments, referrals, and care coordination.
• Plan and coordinate Home Health services with patients, families, physicians, providers, vendors, and facilities.
• Ensure timely start of care and regulatory compliance.
• Review Plans of Care and physician orders for clinical appropriateness.
• Conduct regular OASIS reviews for accurate documentation.
• Provide coaching and support to direct care staff and assist with interviewing.
• Collaborate with Branch Director to monitor KPIs and support operations.

Job Description:

As a Clinical Team Manager, you'll contribute to our success in the following ways:

• Oversee patient care services, staff assignments, referrals, and care coordination to ensure patient needs are met.
• Plan and coordinate Home Health services with patients, families, physicians, providers, vendors, and facilities.
• Ensure timely start of care and ongoing compliance with applicable regulatory guidelines.
• Review and approve individualized Plans of Care, physician orders, and proposed changes for clinical appropriateness.
• Conduct regular OASIS reviews to support accurate documentation and quality patient care.
• Provide ongoing guidance, coaching, and support to direct care team members and assist with interviewing and hiring clinical staff.
• Partner with the Branch Director or Administrator to monitor key performance metrics and support clinical operations, including field work as needed.

What is Required?

1. Associate degree in a Nursing related field, Bachelor preferred.
2. 2 years of home care experience in Home Health or Hospice environment.
3. 1 year of supervisory experience required.
4. Current, unrestricted RN license valid for the state of work.
5. Ability to quickly become proficient in enterprise applications such as Homecare Homebase, Workday, and have knowledge in Microsoft Office.
6. Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
7. Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
