Job summary

Affinity Hospice Holdings, LLC in Alabama seeks an Administrative Records Specialist to support the IDG clinical team, handling medical records, scheduling, QA, and IDG documentation. The role emphasizes accuracy, compliance with state/federal/ACHC rules, and confidentiality.

The position requires an LPN or nursing degree, 1 year in clinical records management (hospice preferred), and solid information systems knowledge. Full-time, 40 hours weekly, on-site with opportunities for advancement.

Qualifications

  • Diploma/Degree in nursing with an LPN or related field from an accredited college
  • Licensed Practical Nurse (LPN) is preferred
  • One year of clinical records management experience, preferably in hospice care
  • Information systems knowledge required
  • Accredited Records Technician Certification preferred

Responsibilities

  • Manage all daily clinical records functions including establishing and implementing clinical records policies
  • Ensure clinical record systems are maintained in compliance with state, federal, and ACHC regulations
  • Maintain comprehensive working knowledge of state, federal, and ACHC documentation regulations and serve as a resource for organization personnel
  • Protect all clinical records through control procedures for open and closed records
  • Protect confidentiality of patient and organization information through effective controls and supervision of clinical records
  • Forward copies of clinical records to authorized users according to policy
  • Complete an administrative record audit after patient discharge and forward to Clinical Manager for audit
  • Monitor aged unsigned physician orders and submit second/third requests for signed orders to overdue physicians
  • Call or visit offices of seriously overdue physicians
  • Supervise the clinical records information system and oversee upgrades
  • Recruit, select, orient, and manage members of the clinical records team
  • Manage scheduling support to ensureCoverage for patient plans of care and IDG coordination
  • Collaborate with clinical, administrative, and leadership teams to align scheduling with patient and family needs
  • Perform other Clinical Support duties as assigned to support department goals
  • Demonstrate collaboration with IDG members
  • Knowledge of corporate business management, regulations, and hospice operation standards
  • Demonstrate good communication and public relations skills
  • Demonstrate autonomy, organization, assertiveness, flexibility and teamwork

Position Summary:

Responsible for providing direct and comprehensive administrative support to the hospice IDG clinical team, which encompasses medical records, scheduling, QA activities, and IDG documentation support.

Hours / Schedule:

Full Time; (40) hours / week.

Minimum Qualifications:
  • Diploma/Degree in nursing with a Licensed Practical Nurse (LPN) or related field from an accredited college or university or the international equivalent is preferred.
  • Licensed Practical Nurse (LPN) is preferred.
  • One (1) year experience in clinical records management, preferably in hospice care operations.
  • Information systems knowledge required.
  • Accredited Records Technician Certification preferred.
Key Responsibilities:

  • Forward copies of clinical records to authorized users according to policy.
  • Complete an administrative record audit following patient discharge and forward the record to the Clinical Manager for clinical audit.
  • Monitor aged unsigned physician orders and submit second and third requests for signed orders to overdue physicians.
  • Call or visit the offices of seriously overdue physicians.
  • Supervise the use of the clinical records information system and maintain a comprehensive working knowledge of the system including upgrades and enhancements.
  • Recruit, select, orient, and manage members of the clinical records team.
  • Manage the members of the clinical record team to ensure accurate and prompt filing and retrieval of clinical records.
  • Responsible for providing direct support and assistance related to scheduling activities as assigned Work directly with clinical, administrative, and leadership team members to ensure scheduling assignments are established, communicated, and coordinated to support the needs of patients and families as outlined via the Plan of Care and IDG.
  • Perform other Clinical Support duties as assigned by management to support department goals and initiatives.
  • Demonstrate the ability to collaborate with an IDG member.
  • Knowledge of corporate business management, governmental regulations and general hospice operation standards.
  • Demonstrate good communication skills and public relation skills.
  • Demonstrate autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.
