Job summary

Affinity Hospice is seeking a Clinical Manager to oversee the coordination, delivery, and management of hospice clinical services. The role ensures high-quality patient care and regulatory compliance while leading a team of clinicians.

Candidates must be a Registered Nurse with BSN, have hospice leadership experience, and be able to market services to the community. This full-time position offers comprehensive benefits and growth opportunities.

Qualifications

  • Registered Nurse with current license in GA.
  • BSN from NLN-accredited program.
  • Three years leadership management in hospice.
  • Two years direct hospice field experience.
  • Supervise clinical/admin personnel.
  • Ability to market and engage with customers/community.
  • Valid driver's license, reliable transportation, and auto liability insurance.
  • Ability to lift and move patients per safety requirements.
  • Knowledge of hospice Medicare/state regulations and ACHC/JCAHO standards.

Responsibilities

  • Oversee assessment, planning, implementation, and evaluation of patient care.
  • Monitor census trends and adjust staffing, revenue, and expenses.
  • Assist in hiring, evaluation, and termination of clinical personnel.
  • Ensure clinical notes, orders, and processes meet policy and quality standards.
  • Coordinate orientation and continuing education for staff.
  • Lead performance improvement activities and audits.
  • Ensure compliant operations and licensure with ACHC/JCAHO standards.
  • Collaborate with intake and marketing to accept referrals.
  • Schedule on-call, weekend, and vacation coverage.
  • Respond to patient complaints with service-first approach.

Position Summary

The Clinical Manager is responsible for ensuring that patient care is coordinated, delivered and managed appropriately. The Clinical Manager is responsible for the overall direction of hospice clinical services.

Hours / Schedule

Full Time; (40) hours / week.

Minimum Qualifications
  • Registered Nurse with current licensure to practice professional nursing in the state where agency located
  • Bachelor's degree in nursing from an accredited program by the National League for Nursing
  • Three years of leadership management experience in a hospice or related health care organization
  • Two years' experience in a direct Hospice field
  • Demonstrated ability to supervise and direct professional administrative personnel
  • Ability to market and deal tactfully with customers and the community
  • Must have valid driver's license, reliable transportation, and auto liability insurance
  • Using effective body mechanics, must be able to lift 40 lbs. floor to waist; lift 10 lbs. waist to shoulder; lift and carry 40 lbs. and push/pull 40 lbs.
  • Must be able to (or assist with) lift, turn, move and position patients
  • Must be able to ensure clinically competent/qualified staffing is appropriate to meet the needs of the patients served
  • Must be able to ensure efficient, appropriate, and cost-effective clinical operations of the agency
  • Must be able to modify/approve clinical paperwork, as well as perform chart audits and provide associate education when necessary
  • Must have knowledge of hospice Medicare, state regulations, governmental regulations, ACHC, and JCAHO standards (if applicable)
Key Responsibilities
  • Provides help in assessment, planning, implementation and evaluation of patient and family/caregiver care to all clinical personnel as indicated
  • Interprets operational indicators to detect census changes and increases or decreases in volume, which could impact staffing levels, revenues or expenses
  • Assists in hiring, evaluating, and terminating clinical personnel
  • Oversees and is responsible for the content and compliance of every clinical note, order, practice and process produced or performed by the clinical staff.
  • Responsible for orientation of new organization personnel.
  • Participates in the planning, development and implementation of in-services and continuing education programs to meet education and training needs of organization personnel
  • Assists with the evaluation of organization performance via performance improvement program, productivity, quarterly and annual reviews.
  • Assures for the quality and safe delivery of hospice services provided through the Organization
  • Assures compliance with all local, state and federal laws regarding licensure and certification of organization personnel and, maintains compliance to the ACHC Hospice Care standards
  • Promotes hospice referrals in the health care community
  • Responsible for holding clinical staff accountable for meeting all productivity and quality standards.
  • Performs joint visits with associates at least semi-annually and documents their performance
  • Coordinates the on-call schedule, weekend schedule, and vacation schedule for all clinical staff.
  • Ensures patient care is performed according to agency policy and regulatory/state requirements
  • Responds to patient complaints in a timely and thorough manner with customer service as top priority.
  • Documents complaints and communicates with associates in response to complaints as appropriate.
  • Assists in auditing of charts in response to Adverse Event and OBQI reports.
  • Participates in regulatory agency reviews
  • Accountable to counsel and develop associates in the meeting of their job expectations
  • Works cooperatively with intake and marketing staff in accepting new referrals
  • Performs visits and admissions as needed based on staffing availability and coverage during vacations or illness
  • Collects/trends all required data for PI program utilizing appropriate forms.
  • Submits PI quarterly results to Compliance and Quality Manager
  • Other duties as assigned by the Executive Director
