Job summary

Eventus WholeHealth is seeking a Clinician Support Specialist to assist the clinical care team and coordinate workflows in skilled nursing and assisted living facilities. You will work with Advanced Practice Clinicians and facility staff to enhance patient care, documentation, and communication.

In this role, you will manage schedules, support encounter documentation, and help with telehealth visits while upholding high standards of patient privacy and professionalism in a fast-paced clinical

Qualifications

• Minimum one year of patient care experience preferred
• Medical scribing experience preferred
• Valid driver’s license and reliable transportation

Responsibilities

• Prepare for daily clinical rounds by ensuring all necessary patient information, documentation, and schedules are ready in advance.
• Support clinicians in delivering care by assisting with encounter documentation, data entry, and chart preparation.
• Coordinate and maintain accurate patient schedules, including updates for admissions, discharges, and acute changes.
• Serve as a liaison between clinicians, facility staff, patients, and families to support communication and quality care delivery.
• Assist with triage needs and documentation as directed by the clinician.
• Organize and manage clinical documentation, including lab results, diagnostic data, and patient records.
• Host and support telehealth visits for both routine and acute care needs.
• Obtain and process patient consents and ensure proper routing of documentation.
• Participate in facility meetings and communicate updates to the care team.
• Support discharge planning and ensure accurate patient census and records.
• Maintain professionalism and represent the organization positively within partner facilities
