EventusWholeHealth is expanding its clinical support team to enhance care coordination across skilled nursing and assisted living facilities. The Clinician Support Specialist will work with Advanced Practice Clinicians and facility staff to streamline workflows, document encounters, and support patient communications.

This role requires strong organization, attention to detail, and the ability to thrive in a fast-paced healthcare environment.

Qualifications

• High school diploma or equivalent; CMA/CNA/PCT certification preferred.
• Experience in patient care and the ability to support clinicians in a fast-paced setting.
• Strong communication and organizational skills are essential.

Responsibilities

• Prepare for daily clinical rounds with patient information and schedules.
• Assist clinicians with encounter documentation, data entry, and chart preparation.
• Coordinate and maintain patient schedules, admissions, discharges, and changes.
• Serve as liaison between clinicians, facility staff, patients, and families.
• Assist with triage needs and documentation as directed by the clinician.
• Organize and manage clinical documentation including lab results and records.
• Host and support telehealth visits for routine and acute care needs.
• Obtain and process patient consents and route documentation properly.
• Participate in facility meetings and communicate updates to the care team.
• Support discharge planning and ensure accurate census and records.
• Maintain professionalism and represent the organization positively.
