Aveanna Healthcare is seeking a Nurse Supervisor to lead clinical staff in Fort Wayne, IN. The role focuses on internal case management, start of care processes, and mentoring nurses in home settings.

The position requires an active RN license and at least 3 years of hands‑on nursing experience, with pediatric and home care background preferred. Travel to patients’ homes is required, with hybrid work flexibility depending on branch needs.

• Willingness to travel to patients' homes in designated territory.

Responsibilities

• Internal case management, evaluation, education and development of nursing talent.
• Transition patients from facility to home through Start of Care assessments and re-evaluations.
• Review physicians' orders and update plans of care and documentation as needed.
• Assist with selecting, training and validation of new clinical staff.
• Promote and enhance clinical expertise to provide safe, quality nursing care.
• Collaborate with operations and business development teams.
• Ensure adherence to State, Federal, Local regulations and Aveanna policies.

Clinical Supervisor (RN) (Nursing Supervisor)

Responsibilities Of The Nursing Supervisor

• Internal case management, evaluation , education and development of nursing talent
• Participating in the transition of our patients from the facility to their home through "Start of Care Assessments" and "Re-evaluations"
• Review physicians' orders and update the plans of care and documentation, as necessary
• Assist with selecting, training and validation of new clinical staff
• Promote and enhance the level of clinical expertise of staff to provide safe and quality nursing care for our patients
• Collaborating with the operations and business development team
• Ensure adherence to State, Federal, Local regulations and Aveanna policies and procedures

Qualifications Of The Nursing Supervisor

• Must have good standing license (RN) in the state in which the clinician will practice- KS, MO or compact license
• Minimum 3 years of hands-on nursing experiencepreferred
• Previous pediatric and home care experience strongly preferred.
• Must be able to travel to patients' homes in designated territory
• Must have reliable transportation, valid driver's license, and pass MVR check
• Current CPR certification from AHA or ARC
• Must be willing to supervise caregivers providing care to infants, children, adolescents and/or adults
