BAYADA Home Health Care, Inc. in Williston and Rutland, Vermont is seeking a Clinical Manager (RN) to lead our home health team. The role combines clinical expertise with leadership to support field staff and deliver patient-centered care in clients’ homes.

As Clinical Manager, you’ll oversee care planning, mentor clinicians, and ensure high-quality outcomes for adult patients while promoting professional growth and collaboration across the care team.

Qualifications

• Current Registered Nurse (RN) license.
• Home Health Care experience preferred.
• Strong clinical assessment, communication, organization, and problem-solving skills.
• Ability to work collaboratively with clients, families, and an interdisciplinary care team.

Responsibilities

• Lead and inspire field nursing staff, fostering a patient-focused team culture.
• Perform client visits and nursing assessments in Williston and Rutland service areas.
• Develop individualized plans of care and monitor implementation to ensure quality outcomes.
• Provide clinical guidance, supervision, and ongoing support to nurses in the field.
• Facilitate new-hire orientation and support onboarding of new team members.
• Promote nurse training, staff education, and in-service programs to strengthen clinical skills.
• Maintain high standards of clinical practice, patient safety, and compliance.

What You'll Do as a Clinical Manager

As a Clinical Manager, you’ll combine hands‑on clinical expertise with leadership and mentorship to help your team deliver exceptional home health care.

• Lead and Inspire: Motivate, mentor, and support field nursing staff while fostering a collaborative, patient‑focused team culture.

• Deliver Clinical Excellence: Conduct client visits and nursing assessments throughout the Williston and Rutland service areas and surrounding communities.

• Oversee Care Planning: Develop individualized plans of care and monitor their implementation to promote quality outcomes and continuity of care.

• Support and Supervise Clinicians: Provide clinical guidance, supervision, and ongoing support to nurses working in the field.

• Welcome New Team Members: Facilitate new‑hire orientation and help nurses feel confident, prepared, and supported in their roles.

• Promote Professional Growth: Participate in nurse training, staff education, and in‑service programs that strengthen clinical knowledge and skills.

• Champion Quality Care: Help maintain high standards of clinical practice, patient safety, and compliance while supporting the needs of clients, families, and staff.

What You'll Need
