Aveanna Healthcare – Lancaster, PA seeks a Clinical Supervisor (RN) to lead a team of in-home caregivers, ensure safe, high-quality, and compassionate home health care. This role oversees care plans, coordinates patient care, and maintains compliance with CHAP and state regulations.

In-office and in-home visits are included, with on-call rotation and collaboration with care teams. Ideal candidates hold an active RN license, a nursing degree (ADN/ Diploma/ BSN), and valid driver's license with

Qualifications

• RN license active in applicable state.
• Associate’s, Diploma, or Bachelor’s in Nursing.
• Valid driver’s license and acceptable MVR.
• Clinical experience per state/federal guidelines.

Responsibilities

• Develop and oversee care plans from admission to discharge.
• Conduct in-home supervisory visits and provide direct care as needed.
• Ensure timely documentation and communicate care updates.
• Train, mentor, and evaluate clinical staff.
• Participate in hiring and performance management.
• Support a positive and growth-oriented team environment.
• Ensure regulatory and accreditation compliance (e.g., CHAP).
• Monitor service quality and implement improvements.
• Promote safety and ethical care practices.
• Participate in on-call rotation.
• Collaborate with care teams and external partners.
• Maintain equipment and medical supplies.

Position Overview

The Clinical Supervisor (RN) leads a team of caregivers to ensure safe, high-quality, and compassionate care in the home setting. Responsibilities include supervising staff, coordinating patient care, maintaining compliance, and supporting clinical excellence.

Key Responsibilities

Care & Oversight

Physical & Work Requirements

• Ability to lift up to 40 lbs; walk, bend, kneel, and climb stairs
