Job summary

STAT Home Health in Baton Rouge, LA is seeking a Clinical Supervisor RN to lead daily operations of district nursing teams and ensure high-quality home health services. A Louisiana RN license and home health experience are preferred.

You will coordinate staff, mentor nurses and aides, ensure regulatory compliance, and support patient care through case management and performance evaluations. ESOP benefits are included.

Qualifications

• Currently licensed RN in Louisiana.
• Home Health regulatory knowledge required.
• Experience with Home Care Home Base and PointCare.

Responsibilities

• Coordinates and schedules daily workload of district staff to maximize capacity.
• Provides daily supervision of district nurses and Home Health Aides to ensure quality and standards.
• Performs case management of patient load as necessary.
• Appraises staff performance during probation and annually with Nursing Director.
• Evaluates client needs and appropriateness of home care by reviewing intake forms.

Essential Duties

• Coordinates and schedules the daily workload of each district utilizing Agency personnel to their fullest capacity
• Provides daily supervision of district nurses and Home Health Aides to ensure the quality of services and maintenance of standards.
• Performs Case Management of patient load as necessary
• Appraises staff performance during their probationary period and annually with the Director of Nursing.
• Identifies and evaluates client needs and appropriateness of home care by reviewing the initial intake screening form before assignment of direct care staff

Qualifications

• Current Louisiana Registered Nursing License (RN)
• Home Health experience with strong understanding of regulatory requirements
• Home Care Home Base and Pointcare experience required
• Direct patient care service leadership experience
