Job summary

21st Century Home Health Services (21HHS) is seeking a Clinical Supervisor - Therapy. The role balances field visits with on-site leadership, supervising clinicians delivering in-home care and coordinating patient plans.

Based primarily in Walnut Creek with field visits, you will guide care teams and ensure timely support for field clinicians. You will oversee training, case conferences, and care plan revisions, contributing to high-quality home health outcomes in a growing organization.

Qualifications

• Two years management or supervisory experience in a rehabilitation therapy department or program.
• Degree from a baccalaureate or masters program in applicable discipline.
• Current CA Physical Therapist license and BLS certification.
• Valid CA Driver's License with acceptable driving record.

Responsibilities

• Ensure patient care is coordinated and managed appropriately in the field.
• Serve as first contact for field clinicians to provide timely support.
• Oversee clinicians providing in-home patient care and supervise field visits.
• Balance field work with on-site duties at the Walnut Creek office.
• Lead training and education programs for nurses in the field.
• Conduct bi-weekly case conferences and update care plans as needed.
