Job summary

21st Century Home Health Services is seeking a Clinical Supervisor - Physical Therapy to lead field clinicians and ensure coordinated in-home patient care. This hybrid role combines field visits with on-site work at the Walnut Creek office, with 2 days in the office and 3 days in the field, supervising 12 patient visits weekly.

Qualifications

• Two years management or supervisory experience in a rehabilitation therapy department or program.
• Degree from a bachelor's or master's program in applicable discipline approved by an accredited organization.
• Current CA Physical Therapist license.
• Current Basic Life Support (BLS).
• Valid CA Driver's License with acceptable driving record.
• All licenses and certifications must be current at time of hire and sustained throughout employment.

Responsibilities

• Oversee field clinicians delivering patient care in patients' homes.
• Coordinate training and educational programs for nurses in the field.
• Serve as the first point of contact for field clinicians and assist in patient care escalations.
• Lead bi-weekly Case Conferences with the clinical team to discuss patient progress and care plan revisions.
• Review cases and guide clinicians to improve performance and delivery of home care services.
