21st Century Home Health Services (21HHS) is seeking a Clinical Supervisor - Occupational Therapy to oversee field clinicians delivering home-based care. The role ensures coordinated and timely patient care in residences, with leadership responsibilities across the Sacramento area.

Qualifications

• Two years of management or supervisory experience in rehabilitation therapy, preferably home care.
• Degree from a baccalaureate or master's program in an approved discipline.
• Current CA Occupational Therapist license.
• Current Basic Life Support (BLS).
• Valid CA Driver's License with acceptable driving record.

Responsibilities

• Oversee field clinicians providing care to patients in their homes.
• Act as first point of contact for field clinicians to ensure timely support.
• Coordinate training and educational programs for nurses in the field to improve outcomes.
• Assess staff learning needs and set goals for the next evaluation period.
• Lead bi-weekly case conferences with the clinical team to revise care plans as needed.
• Review and evaluate cases to promote effective performance and quality home care services.
