Multiple Full-Time Clinical Supervisor: Nursing positions available

Coverage areas: Santa Clara County to Hayward/Fremont and Santa Clara County to Santa Cruz

Reporting to the Director of Nursing, you will oversee a team of RN Case Managers and LVNs, ensuring coordinated and effective patient care. You will serve as the first escalation point, support staff development through educational programs, and maintain client relationships by providing updates and insights to improve patient outcomes and reduce readmissions.

This role involves a hybrid of fieldwork and office work at the 21HHS San Jose office:

• Office: Spend 2 days per week on administrative and team management duties.
• Fieldwork: Spend approximately 3 days per week (average 12+ hours) conducting supervisory visits and supporting clinicians in the field.

Responsibilities include:

• Overseeing clinical care delivery in patients' homes.
• Handling patient and client communications promptly.
• Coordinating training and educational initiatives for field nurses.
• Assessing staff learning needs and setting development goals.
• Serving as the primary contact for clinicians regarding patient care escalations.
• Conducting bi-weekly case conferences to review patient progress and adjust care plans.
• Reviewing clinical cases, procedures, and documentation to ensure quality and compliance.
• Assisting clinicians in establishing care goals and developing plans of care.

Qualifications for success:

• At least two years of management or supervisory experience in healthcare within the past five years, preferably in home health.
• Graduate of an accredited nursing program; Bachelor’s degree preferred.
• Current CA RN license.
• Current BLS certification.
• Valid CA driver’s license with a clean driving record.

Compensation: $143,000 - $163,000 annually, plus a quarterly bonus of $40,000. Starting salary depends on experience. We also offer bonus incentives, comprehensive health benefits, and a 401k with up to 4% match.
