CareStar, Inc. in Hamilton, OH is seeking a Clinical Supervisor to lead Ohio Home Care operations across Hamilton and surrounding counties.

This exempt role blends remote work with field visits, and you will supervise care coordination, assessments, and service plan development to support at-risk individuals. The ideal candidate is licensed as an RN or social worker (LSW/LISW) with at least five years of clinical home-and-community base experience and one year of supervisory practice, plus CCM

Qualifications

• Experience providing care management/coordination preferred.
• Must be either licensed RNs, licensed social workers or licensed independent social workers (LSW, LISW) with at least five (5) paid years of clinical experience in home-and community based services and a minimum of one (1) year of supervisory experience.
• Certified Case Manager (CCM) required or obtained within one year of being in the position.
• Experience in completing assessments, service plan development and monitoring.
• Effective organizational and communication skills.
