Job summary

First Day Homecare in San Antonio, TX is seeking a Clinical Supervisor (RN) to lead client care teams and ensure quality homecare services. You will supervise staff, conduct home visits, and coordinate care within the agency's policies and Nurse Practice Act.

You will report to the Director of Clinical Services and support staff through training, chart reviews, and continuous improvement efforts. A valid RN license and active CPR certification are required.

Qualifications

• Graduate of an accredited school of professional nursing; BSN preferred.
• Current RN license in practicing state; 1 year in last 3 years and 2 years homecare with 1 year supervisory experience.
• Excellent verbal and written communication; strong interpersonal skills.
• Licensed driver with insured automobile; CPR certification current.

Responsibilities

• Conduct in-home client visits for start-of-care assessments, supervisory visits, and reassessments.
• Be available during business hours and on-call as needed to support quality care delivery.
• Receive referrals, assess service needs, and assign clinicians to cases.
• Meet with clinical staff to provide guidance, set goals, and evaluate progress.
• Attend clinical meetings to coordinate care and participate in on-call rotation.
• Guide staff, clients, and families on healthcare resources and clinical policies.
• Oversee accuracy and timeliness of clinical documentation and perform quarterly chart reviews.
• Assist with staff hiring, orientation, training, and annual performance evaluations.

Schedule / Expected Work Hours:

• Monday through Friday, 9:00a - 5:00p.
• The position may require occasional weekend or evening visits to client homes and participation in a clinical on-call rotation (phone support only).

Job Summary

Manages, supervises, coordinates, evaluates, and develops client care teams to ensure quality care delivery within the scope of services of the Agency and all applicable policies, standards, laws, and state Nurse Practice Act.

Responsibilities

1. Conduct in-home client visits such as start-of-care assessments, supervisory visits, and reassessment visits.
2. Be available during business hours and on-call as needed to support quality care delivery.
3. Receive referrals, assess service needs, and assign appropriate clinicians to cases.
4. Meet regularly with clinical staff to provide guidance, set goals, and evaluate progress.
5. Attend clinical meetings to facilitate care coordination and participate in on-call rotation.
6. Support staff, clients, and families by providing direction on healthcare resources and educating them on clinical policies.
7. Oversee accuracy and timeliness of clinical documentation and perform quarterly chart reviews.
8. Assist with staff hiring, orientation, training, and conducting annual performance evaluations.

Qualifications

• Graduate of an accredited school of professional nursing. BSN preferred.
• Current license as a Registered Nurse in the state(s) of practice having one (1) year nursing experience within the last three (3) years, at least two (2) years of homecare experience and one (1) year of supervisory experience.
• Excellent verbal and written communication skills and strong interpersonal skills.
• Must be a licensed driver with an automobile insured in accordance with Agency and state requirements.
• Current CPR certification.
