Job summary

First Day Homecare in New Jersey seeks a registered nurse supervisor to lead and develop client care teams, ensuring safe, compliant home health services across the agency.

You will oversee clinical staff, coordinate referrals, participate in on-call rotation, and ensure adherence to nursing standards and state regulations while supporting families and patients in their homes. This role requires 30–40% travel and occasional evenings or weekends. Reports to the Director of Clinical Services.

Qualifications

• Graduate of an accredited school of professional nursing with RN licensure in NJ.
• Two to three years of home care experience depending on degree.
• One year of supervisory experience and excellent communication skills.

Responsibilities

• Direct and supervise clinical staff to ensure safe, effective home care.
• Be available during business hours and as needed for support.
• Receive referrals, assign clinicians, and ensure plan of care is met.
• Lead regular staff meetings, set priorities, and evaluate progress toward goals.
• Attend case conferences and participate in on-call rotation with phone support.
• Oversee client admissions, chart reviews, and documentation accuracy.

Schedule / Expected Work Hours:

• Monday through Friday, 9:00a - 5:00p.
• The position will require 30% to 40% travel.
• The position will require occasional weekend or evening visits to client homes and participation in a clinical on-call rotation (phone support only).

Responsibilities

1. Manages and directs a team of clinical staff consisting of RNs, LPN/LVNs, and Home Health Aides, ensuing safe, effective, and appropriate home care services.
2. Is available at all times during regular business hours and as needed to provide support and assure quality care delivery to home care clients.
3. Receives referrals, determines services required, and Agency’s ability to meet needs. Assigns appropriate clinicians to cases.
4. Meet with clinical staff on a regular basis to provide guidance and information related to specific issues. Provides direction to teams to assure that client needs are met and services are provided according to the plan. Assists clinical staff in establishing priorities, setting goals, and evaluating progress toward goals.
5. Attends case conferences and other clinical meetings to facilitate coordination of care.
6. Participates in clinical on-call rotation as requested by the Agency, providing telephone support to clients as needed.
7. Completes client care visits or shifts as directed by the Director of Clinical Services.
8. Provides support and direction to Agency staff, other health care professionals, clients, and families related to appropriate and available health care resources.
9. Educates Agency staff on clinical services, policies, and procedures as needed.
10. Ensures accuracy, completeness, and timeliness of Agency clinical documentation on a weekly basis.
11. Completes quarterly, or more frequent as needed, client chart reviews.
12. Assists in screening, interviewing, orientation, and training of new Agency personnel.
13. Conducts timely annual performance evals on clinical staff.
14. Performs client admissions, supervisory visits, and reassessments as directed by Director of Clinical Services.
15. Develops care plans as appropriate to each client.
16. Manages medical practitioner orders for care and treatment of clients, where applicable.
17. Performs other duties as assigned.

Qualifications

• Graduate of an accredited school of professional nursing. BSN preferred.
• Current license as a Registered Nurse in the state(s) of practice with the following experience at a minimum: Two (2) years' homecare experience if bachelor prepared or three (3) years' homecare experience if non-bachelor prepared; and one (1) year supervisory experience.
• Excellent verbal and written communication skills and strong interpersonal skills.
• Must be a licensed driver with automobile insured in accordance with Agency and state requirements.
• Current CPR certification.
