Job summary

Palms Home Care is seeking a Clinical Supervisor to lead and support the clinical team, ensuring high-quality patient care and adherence to regulatory standards.

You will mentor staff, oversee care plans, manage performance, and collaborate with the Clinical Director to drive excellence.

Qualifications

• RN licensure in good standing.
• At least one year of management or supervisory experience in the home health setting. Strong leadership and interpersonal skills.
• Familiarity with OASIS, care documentation requirements, and interdisciplinary coordination.
• Experience using electronic medical record (EMR) systems such as Kinnser.
• Knowledge of clinical best practices and regulatory standards.
• Commitment to maintaining patient confidentiality and ethical standards.

Responsibilities

• Review patient information related to case and assign clinicians.
• Review patient diagnoses, medications, procedures and clinical course.
• Supervise and mentor clinical staff, including therapists, counselors, and other healthcare professionals.
• Conduct regular performance evaluations and provide constructive feedback to staff.
• Facilitate clinical training and professional development opportunities.
• Collaborate with interdisciplinary teams to develop and implement patient care plans.
• Monitor patient outcomes and ensure adherence to best practices and clinical guidelines.
• Assist in resolving clinical issues and addressing staff concerns.
• Assist in screening and interviewing process of new personnel and make employment recommendations.
• Perform duties as delegated by the Clinical Director.

We are seeking a dedicated and experienced Clinical Supervisor to join our Palms Home Care team. The ideal candidate will provide leadership, guidance, and support to clinical staff to ensure the delivery of high-quality patient care.

Responsibilities

• Reviews available patient information related to case, including disciplines required, to determine home care needs. Assigns appropriate clinicians to case.

• Reviews patient’s clinical diagnosis, medications, procedures and clinical course.

• Supervise and mentor clinical staff, including therapists, counselors, and other healthcare professionals.

• Facilitate clinical training and professional development opportunities.
